The Main Line Health System (MLHS) Nursing, in collaboration with Habit Change, A Whole Life Health Company and in keeping with Well Ahead, is pleased to bring you this exciting program beginning Saturday, May 19, 2012.

Participation in this pilot is limited to the first 15 registrants. A waiting list will be compiled for the next pilot
group(s) beginning in the Fall.

Program at a Glance

1.   Pre-Program Assignments

Participants will receive information on the timing of two pre-program assignments. One is an online Assessment, including 8 habit areas. The second assignment will be the completion of a physical examination administered by Cardio-Kinetics. The physical examination will occur 3-4 weeks prior to the beginning of the 6-session education program allowing time for the Assessment and analysis.

2.   Assessments

The completion of Assessments takes approximately 3-4 hours. Through the Assessments, Habit Change, A Whole Life Health Company and participants get a clear picture of how they understand and regard themselves in the Assessment areas described below. Information derived from the Assessments is then analyzed to develop each the participant’s Starting Point. The Starting Point establishes that the baseline for each person’s Life Plan developed in partnership with the Habit Change Coach.

Assessments:
• Physical Assessment by appointment with Cardio-Kinetics
• Psychological, including Motivation Style
• 8 Habit Area Practices: Resilience, Exercise, Nutrition, Sleep, Learning, Friendship, Simplicity and Spirituality

3.   Habit Change Education

Habit Change, A Whole Life Health Company facilitates an experiential, group education session that includes training in Mindfulness meditation, and lectures by experts on the latest health information on the 8 Habit Areas and the relationships among the Habit Areas.

4.   Program Schedule

o Pre-Program Assessments
On Wednesday, April 18th you will receive an email that contains instructions for the completion of two assignments that you must complete prior to Wednesday, May 2.
      1. On-Line Assessments: The assessments are tools that will help make you and your coach aware of areas of your life that you may want to address. We recommend that you allow 3-4 hours over a period of days to complete these assessments.
      2. Physical Assessment with Cardio-Kinetics: You must schedule and complete a physical provided by Cardio-Kinetics which should take about 45 minutes to complete.

o Welcome Meeting, Wednesday, April 25, 6:30-8 pm, Annenberg Center Room 101A – Lankenau Hospital

o Kickoff Retreat, Saturday, May 19, 2012: 9 am to 1 pm

o 4 Weekly Meetings, Wednesday evenings: 6:30 pm to 9:30 pm:
• Week 1: May 30 - Resilience & Mindfulness
• Week 2: June 6 - Exercise, Nutrition and Sleep
• Week 3: June 13 - Learning and Friendship
• Week 4: June 20 - Simplicity and Spirituality

o Closing Graduation Ceremony - Saturday, June 30, 2012: 9 am to 1 pm
This is a real celebration of the individual and group achievements during the Habit Change Process and is an opportunity for friends and family to participate.

o 2 Reunions: Saturdays, 9 am –Noon - Annenberg Center Room 101A
             3 Month: Saturday, September 29, 2012
             *6 Month: Saturday, January 5, 2013*

5.   Habit Change Coaching

Each participant is matched with a certified Habit Change Coach who will talk with that participant weekly during the first 6 weeks (typically done by phone) for 30 minutes each week. The Coaching relationship continues every other week for 6 months following the Graduation Ceremony.

6.   Reunions

At 3 and 6 months, the group reconvenes for a 2-3 hour reunion meeting that serve as a way to get back together, provide updates on health information, group sharing, data collection and feedback on progress in the Habit Change Process.

Cost

The total program cost, including coaching and 2 reunions, is being significantly underwritten by Habit Change, A Whole Life Health Company, and is valued at more than $1,500.

However, the cost to MLHS Nurses is only $300. Payments can be made by Credit Card and we are offering 2 payment plans:

1.   Full Payment of $300
            – or -
2.   3 payments of $100 ($100 at enrollment, followed by 2 monthly credit card charges of $100)
